A community partnership dedicated to improving the health and well-being of African American and Latino families during pregnancy and early childhood.

A team of community health workers (CHW), nurses, counselors, and nutritionists is available and will schedule a home visit to make sure you are doing well, provide personal support, listen to you, help you set goals, share information, and connect you to community resources.

When a CHW looks and sounds like you, a woman relaxes a little bit more and can state exactly what she needs.

We recently worked with an African refugee client whose goal was to have reliable transportation to make it to appointments. She had missed appointments in the past due to a lack of transportation, language barriers, and not having a working cell phone.

Since her CHW arranged for consistent transportation for appointments, mom has not missed any. Also, it’s been easier for communication since her CHW speaks her language. Plus, her CHW was able to install WhatsApp on her cell phone; an App that uses WI-FI and works even when her phone is out of service.

“We want to partner with you to help you achieve the goals that are important to you. From family support, to health education, all our activities take into account a person’s cultural background and unique circumstances.”
