WHAT WE DO
Give families the tools they need to provide opportunities for their children.
- Visit first-time parents in the hospital and offer a home visit within the first few weeks of their child’s life
- Offer families a menu of services to address the medical, social, intellectual and emotional needs of young children and their caregivers
- Provide a first call for help where families can get immediate answers to their questions as well as referrals to community resources
Help all children access quality healthcare.
- Improve the health and well-being of children in Kent County by connecting families to a quality medical home
